Responsibilities
- Manage and treat patients assigned to your specific geographic region
- Assess patients' physical therapy needs in a home environment and create tailored treatment plans
- Collaborate with physicians, patients, and families to establish care plans and treatment objectives
- Design, organize, and execute customized therapy programs to enhance functionality and encourage self-reliance
- Train family members, PTAs, Home Health Aides, and caregivers in suitable therapy methods
- Ensure accurate and prompt documentation for each patient visit
- Maintain clear communication with patients, families, and interdisciplinary team members, particularly during discharge planning
- Provide care that adheres to agency guidelines and professional standards
