Responsibilities
- Manage and treat patients within a specific geographic area.
- Assess patients' physical therapy needs in their homes and create personalized treatment plans.
- Collaborate with physicians, patients, and families to establish care plans and treatment objectives.
- Design, prepare, and execute tailored therapy programs to enhance function and encourage independence.
- Train family members, PTAs, Home Health Aides, and caregivers in appropriate therapy methods.
- Maintain accurate and timely documentation for each visit.
- Communicate effectively with patients, families, and interdisciplinary team members, with a focus on discharge planning.
- Provide care that adheres to agency policies and professional standards.
