Responsibilities
- Provide therapy services to patients within a defined local region
- Assess patient requirements for physical therapy in home environments and create personalized treatment strategies
- Collaborate with physicians, patients, and family members to establish care plans and set therapy objectives
- Design, organize, and carry out individualized rehabilitation programs aimed at restoring functional abilities and encouraging self-sufficiency
- Educate family members, physical therapist assistants, home health aides, and caregivers on proper therapeutic methods
- Ensure prompt and precise documentation for every patient visit
- Maintain clear communication with patients, families, and members of the interdisciplinary team, particularly during discharge planning
- Deliver services in accordance with agency guidelines and professional standards
