Responsibilities
- Manage and treat patients within a specific geographic region
- Assess patients' physical therapy needs in their homes and create personalized treatment plans
- Collaborate with physicians, patients, and families to establish care plans and treatment objectives
- Design, prepare, and execute tailored therapy programs to enhance function and independence
- Train family members, PTAs, Home Health Aides, and caregivers on suitable therapy methods
- Maintain accurate and timely documentation for each visit
- Communicate effectively with patients, families, and the interdisciplinary team, particularly regarding discharge planning
- Provide care that adheres to agency policies and professional standards
Requirements
- Graduate of a Physical Therapist Program
- Licensed Physical Therapist in New York or eligible for licensure
- Ability to travel to patients' homes within the assigned geographic area
