Metro Physical & Aquatic Therapy is hiring a Home Care Occupational Therapist- Part Time (Nassau County)

Responsibilities

  • Serve patients within a designated service region
  • Deliver one-on-one therapeutic treatment sessions
  • Assess patient conditions and create customized therapy plans in accordance with physician referrals and individual needs
  • Administer care that meets professional practice guidelines, organizational policies, and prescribed treatment protocols
  • Instruct family members, home health aides, and other caregivers in appropriate therapeutic methods
  • Design and implement occupational therapy programs aimed at maintaining or improving physical function, reducing pain, and preventing functional decline
  • Conduct and record initial evaluations, analyzing findings to identify issues and establish a clinical diagnosis before beginning treatment
  • Ensure timely and precise documentation for every patient visit
  • Monitor treatment outcomes throughout the care process and modify interventions as needed to optimize patient progress
  • Demonstrate willingness to work with elderly patients
