Metro Physical & Aquatic Therapy is hiring a Home Care Occupational Therapist- 1099 (North Shore, Nassau County)

Responsibilities

  • Treat patients within a designated service area
  • Deliver individualized one-on-one therapy sessions
  • Assess patients and create customized treatment plans in accordance with physician directives and personal needs
  • Administer care that meets professional standards, agency guidelines, and prescribed plans
  • Instruct family members, home health aides, and caregivers in appropriate therapy techniques
  • Design and implement therapy programs to improve, restore, or maintain physical abilities, reduce pain, and prevent occupational decline
  • Conduct and document initial evaluations to analyze patient status, identify issues, and establish diagnoses before treatment begins
  • Maintain accurate and timely visit documentation
  • Monitor treatment outcomes and modify interventions as needed to optimize patient progress

Responsibilities

  • Provide 1:1 treatment
  • Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
  • Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care
  • Educate family members, home health aides, and other caregivers on proper therapy techniques
  • Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
  • Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
  • Provide timely and accurate documentation for each visit
  • Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
