Responsibilities
- Treat patients within a designated service area
- Deliver individualized one-on-one therapy sessions
- Assess patients and create customized treatment plans in accordance with physician directives and personal needs
- Administer care that meets professional standards, agency guidelines, and prescribed plans
- Instruct family members, home health aides, and caregivers in appropriate therapy techniques
- Design and implement therapy programs to improve, restore, or maintain physical abilities, reduce pain, and prevent occupational decline
- Conduct and document initial evaluations to analyze patient status, identify issues, and establish diagnoses before treatment begins
- Maintain accurate and timely visit documentation
- Monitor treatment outcomes and modify interventions as needed to optimize patient progress

Responsibilities
- Accept and treat patients within your designated geographic area
- Provide 1:1 treatment
- Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
- Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care
- Educate family members, home health aides, and other caregivers on proper therapy techniques
- Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
- Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
- Provide timely and accurate documentation for each visit
- Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
