Spring Health is hiring a Global Care Navigator - Italy (Remote)

Responsibilities

  • Assist individuals in Italy with mental health resources and care coordination.
  • Coordinate care with mental health providers and other relevant parties.
  • Provide education and support to individuals and their families.
  • Ensure individuals receive appropriate mental health services.
  • Maintain accurate and up-to-date records of all interactions and care plans.
  • Collaborate with the care team to develop and implement care plans.
  • Identify and address barriers to care and access to services.
  • Provide culturally sensitive care and support.
  • Advocate for individuals and their families to ensure their needs are met.
  • Participate in regular team meetings and training sessions.
  • Stay up-to-date with the latest mental health resources and best practices.
  • Ensure compliance with all relevant laws and regulations.
  • Provide feedback and suggestions for improving care and services.
  • Maintain confidentiality and privacy of all individuals.
  • Work collaboratively with other team members and stakeholders.
  • Provide support and guidance to individuals and their families.
  • Ensure individuals have access to necessary mental health services.
  • Coordinate care with other healthcare providers and organizations.
  • Provide education and resources to individuals and their families.

Nice to Have

  • Master's degree in social work, psychology, or a related field.
