Responsibilities
- Deliver structured, skill-focused individual and group sessions for caregivers enrolled in treatment.
- Apply motivational techniques to support caregivers in following their treatment plans, tracking progress and completion of required activities.
- Join regular interdisciplinary team check-ins to coordinate care and monitor patient advancement.
- Practice empathetic listening and assist caregivers in foreseeing obstacles and preparing coping strategies in advance.
- Maintain an updated availability schedule to allow caregivers to book appointments.
- Initiate follow-ups via calls or messages, recover missed appointments, send reminders, track engagement, and offer support to maintain consistent participation.
- Collaborate with caregivers to solve barriers to treatment adherence, offering practical solutions to reduce daily stressors.
- Personalize care plans by recommending program adjustments and arranging services such as additional clinical sessions, group sign-ups, customized grocery orders, and tailored resource referrals.
- Support caregivers in creating advocacy documents, such as letters requesting 504 Plan accommodations.
- Establish a respectful, trusting relationship with each caregiver.
- Complete accurate and timely records of all caregiver interactions in accordance with organizational standards.
- Attend required internal meetings and complete assigned initial and ongoing training programs.
- Carry out additional tasks as directed by supervisors.

Compensation
Earning Potential Range: $54,000 to $65,000 Base Pay + 15% Annual Performance-Based Bonus Potential

- Work is conducted entirely from home with no travel obligations.
- Position involves stationary work requiring use of standard office equipment, keyboards, and telephone communication.
- Ability to sit or stand as needed during work hours.
- Must be available for 33 hours per week of direct patient care, including groups, new patient intakes, and follow-ups.
- Required to be available until 8 p.m. on two evenings per week, chosen by the employee.
- Must commit to at least 20 hours of direct patient care after 2 p.m. in the patient's time zone each week.
- Required to attend mandatory meetings such as supervision, treatment huddles, and all-hands sessions.
