Responsibilities
- Act in alignment with the agency's mission and guiding principles in all interactions.
- Provide continuous support to help individuals become active participants in self-care, health, safety, household tasks, social activities, and overall well-being, ensuring a high quality of life.
- Maintain a safe and clean home by assisting individuals with household chores and participating as needed.
- Follow all treatment plans and use adaptive and safety equipment to promote independence and ensure health, safety, and well-being.
- Teach meal preparation skills including diet adherence, portion control, and cooking techniques.
- Administer medications following AMAP policies and procedures.
- Implement and document treatment plans and agency materials such as residential habilitation goals, medical case notes, and communication logs to support independence and well-being.
- Adhere to overtime policies to ensure consistent quality support.
- Participate as an interdisciplinary team member in meetings to discuss progress, goals, and share information.
- Provide transportation for medical appointments, shopping, recreational outings, and other needs.
- Mentor new Direct Support Staff by demonstrating routines and reviewing training materials.
- Perform other job-related duties as assigned by the supervisor.
- This role requires regular unsupervised physical contact with individuals receiving services and is subject to fingerprint and criminal background checks.
Requirements
- Valid New York State driver's license.
- Ability to comprehend and follow written and verbal instructions.
Nice to Have
- High school diploma or equivalent.
- AMAP, First Aid, CPR, and SCIP-R training.
- At least one year of experience supporting individuals with intellectual and developmental disabilities.
Other
- Physical aspects may include lifting and transferring individuals (children and/or adults).
- Acting as a second staff support during transportation, including wheelchair vehicle lockdowns.
