About the Role
This role involves delivering high-acuity nursing care to paediatric patients within their homes and community environments, coordinating with families and healthcare professionals to manage long-term conditions and support health outcomes.
Responsibilities
- Deliver clinical care to children with complex medical needs in home settings
- Collaborate with families to develop and implement individualised care plans
- Monitor patient health and adjust care strategies as needed
- Provide education and training to parents and caregivers
- Work alongside GPs, specialists, and therapists to ensure coordinated care
- Maintain accurate and up-to-date patient records
- Respond to urgent clinical situations as part of an on-call rotation
- Administer medications and treatments according to protocols
- Support transitions between hospital and home care
- Participate in multidisciplinary team meetings and case reviews
- Ensure compliance with infection control and safety standards
- Conduct developmental and health assessments
- Advocate for patient and family needs within the healthcare system
- Contribute to service improvement initiatives
- Uphold safeguarding policies and child protection procedures
- Provide end-of-life care when required
- Support siblings and family members affected by a child’s condition
- Use telehealth tools for remote monitoring and consultations
- Attend training to maintain clinical competencies

The Complex Care service supports children and young people with significant health challenges who require ongoing nursing input. The team operates across Dartmouth and surrounding areas, delivering care in homes, schools, and community clinics. Nurses work closely with families to promote independence and improve quality of life.
