About the Role
The specialist will work directly with clinical teams to improve care delivery by identifying opportunities for quality improvement and facilitating adoption of best practices through targeted communication and relationship-building.
Responsibilities
- Engage with primary care providers to understand practice needs
- Use clinical data to identify gaps in patient care
- Support care teams in implementing quality improvement initiatives
- Conduct outreach to providers using evidence-based strategies
- Build trust with clinical staff to foster long-term partnerships
- Translate data insights into actionable care recommendations
- Assist practices in meeting regulatory and performance goals
- Coordinate with internal teams to align outreach efforts
- Deliver educational resources tailored to provider needs
- Track outreach outcomes and adjust strategies as needed
- Respond to provider inquiries with clinical context
- Promote adoption of clinical programs through persuasive communication
- Maintain accurate records of all provider interactions
- Identify barriers to care and suggest practice-specific solutions
- Support care coordination across multidisciplinary teams
Nice to Have
- Nursing or clinical certification
- Direct patient care experience
- Background in care management or care coordination
- Experience with population health tools
- Prior work in value-based care environments
