Responsibilities
- Conduct person-centered planning using a specialized facilitation framework to identify client strengths, goals, needs, and desired outcomes, and create a tailored plan.
- Manage budget and spending by translating personalized plans into concrete support needs and advocating for these needs with local budget authorities.
- Oversee project management to coordinate the process, guiding clients and partner organizations effectively.
- Facilitate access to service providers by utilizing NeuroNav's proprietary resources to find and assist providers in implementing person-centered plans throughout the year.
- Contribute to special projects each quarter by applying insights from client work to key company strategies and initiatives.
Requirements
- Bachelor's Degree or equivalent work experience
- Social work or case management experience in the disability or social services field
- Client-facing experience managing multiple relationships simultaneously
- Excellent written and verbal communication skills
- Must have a computer, reliable high-speed internet connection, and a quiet work environment
Nice to Have
- Fluent in Spanish and bilingual (strongly preferred)
- Experience in a performance-based culture with metrics attainment goals (preferred)
- Experience serving in the developmental disability field (preferred)
- Experience with Microsoft Office and Google Suite (preferred)
- Experience in person-centered planning and in the California developmental disability system (preferred)
- Case management experience: 2 years (Preferred)
