Collaborate with clients, families, and carers to assess needs and develop person-centred plans
Establish meaningful objectives and practical strategies using the Positive Behaviour Support Capability Framework, prioritising the removal of restrictive practices in home and community settings
Design, implement, and review support plans grounded in evidence within complex support environments
Ensure each support plan aligns with the individual's personal goals, interests, and strengths
Enhance the skills and confidence of family members, carers, and support networks through guidance and education
