Responsibilities

  • Collaborate with clients, families, and carers to assess needs and develop person-centred plans
  • Establish meaningful objectives and practical strategies using the Positive Behaviour Support Capability Framework, prioritising the removal of restrictive practices in home and community settings
  • Design, implement, and review support plans grounded in evidence within complex support environments
  • Ensure each support plan aligns with the individual's personal goals, interests, and strengths
  • Enhance the skills and confidence of family members, carers, and support networks through guidance and education
