About the Role
The Behavioral Health Care Manager will coordinate patient care, support treatment planning, and ensure continuity across mental health and substance use services. This role emphasizes patient engagement, clinical documentation, and collaboration with providers to improve health outcomes.
Responsibilities
- Coordinate comprehensive care plans for individuals with behavioral health conditions
- Conduct initial patient assessments and ongoing clinical evaluations

- Ensure timely access to mental health and addiction services
